Publication Forms

For your convenience, PDF versions of forms required for publication are provided here. Please return completed forms to the Editor's office at the address given in your decision letter. More information about which forms are required for your particular manuscript is provided below.

• Publication Rights Form  (83KB)
  The Publication Rights Form must be completed and signed in both Sections 1 and 2 by the corresponding author. All authors must sign in Section 2. The corresponding author must provide an original signature. Co-authors may supply faxed signatures or photocopies.

• Full Disclosure of Interests Form  (38KB)
  A completed Full Disclosure of Interests Form is required for all authors. Authors are required to disclose activities and relationships that, if known to others, might be viewed as a potential conflict of interest. Please complete the top and bottom portions of this form.

• Certificate of Compliance with APA Ethical Principles Form  (26KB)
  For authors whose research involved human participants or animal subjects, APA requires author(s) to certify compliance with APA Ethical Principles. Authors whose work does not involve human participants or animal subjects do not need to submit this form.

• Permissions Alert Form (47KB)
  If your paper includes material reprinted or adapted from another source, please review the permissions guidelines in the Permissions Alert Form. All necessary permissions must be secured prior to publication. Please submit this form and any supporting documentation of secured permissions if needed.

Document Deposit Forms

• NIH - Public Access: PubMed Central Deposit form (15KB)
  For authors whose work was funded in whole or in part by the National Institute of Health (NIH), this form needs to be completed to ensure APA will deposit the final accepted manuscript to the National Library of Medicine's PubMed Central (PMC) database. Authors whose work was not funded by NIH do not need to submit this form.

• Wellcome Trust Article Sponsorship Form  (68KB)
  For authors whose work was funded by the Wellcome Trust, this form needs to be completed to ensure APA will deposit will deposit the final, published article to the National Library of Medicine's PubMed Central (PMC) database. Authors whose work was not funded by Wellcome Trust do not need to submit this form.
