Use the links below to download the appropriate set of Eligible Status Application or Accreditation Self-Study Instructions in Microsoft Word format. This format allows you to type directly into the document, using them as a template. It is strongly encouraged you download them in this format if possible. If you do not use Word, you can download the instructions in Adobe PDF format.

Programs should use the self-study instructions for the year during which they will be site visited. This means that accredited programs assigned self-study due dates prior to September 1 should use the current year's self-study instructions (2015). Accredited programs scheduled for a site visit in 2016 and assigned a self-study due date of September 1 (or later, in the event of a delay), should complete the online self-study (2016).

Notes about "Accredited, on contingency" applications for internship and postdoctoral residency programs:

  • Internship and postdoctoral residency programs submitting applications for "Accredited, on contingency" status prior  to Sept. 1, 2015 should use the 2015 CoA Self-Study Instructions .
  • Internship and postdoctoral residency programs submitting applications for "Accredited, on contingency" status after  Sept. 1, 2015 should use the 2016 CoA Applicant Self-Study Instructions .

For Accredited Programs Scheduled for a 2016 Site Visit

Accredited programs scheduled for a 2016 site visit will complete an online self-study. These programs received an email with instructions on how to access the self-study section of the CoA Portal. Access the CoA Portal online.
